As an eternalHealth member, you can earn up to $85.00 in rewards while getting healthier! From January 1 to December 31, 2026, you can earn rewards by completing each recommended health initiative. Be sure to use your rewards before the end of the year, as they will not roll over!

How to Redeem Your Rewards
Redeeming your rewards is easy and convenient. Simply visit your Nations Benefits portal at eternalHealth.nationsbenefits.com and confirm the health initiatives you’ve completed through a self-attestation process. If you are an active member, your rewards will be added to your eternalPlus Benefits Card within 3 business days.
Flexible Reward Usage
Once funded, you can use these rewards for over-the counter (OTC) items and, if eligible, healthy groceries*. You have the flexibility to redeem your rewards through the online Nations Benefits portal, at participating retail stores, or via mail order.
